Understanding Asian Nasal Anatomy
Asian noses, compared to other ethnicities, typically have:
- A low dorsum (bridge) and short nasal bones
- Thicker skin
- Weaker cartilage support
- Broader nasal base or flared alar rims
- Inadequate amount of septal cartilage available for grafting
These differences in nasal anatomy require Asian rhinoplasty to have specialized techniques different from traditional rhinoplasty approaches. (2)
Techniques and Approaches
The biggest difference between Asian rhinoplasty and traditional rhinoplasty approaches is that Asian rhinoplasty focuses on augmentation of the nasal dorsum, while traditional rhinoplasty focuses more on dorsal hump reduction. While the most common practice in Korea and most Asian countries uses alloplastic implants to augment the nasal dorsum, Dr. Zhao uses autologous grafts for nasal augmentation to minimize risks associated with alloplastic implants, such as extrusion and infection. Most often, there are 3 main aspects of Asian rhinoplasty:
- Dorsum/bridge augmentation – In many cases, Asian rhinoplasty involves the use of grafts to create height or support. Typically, the amount of septum available is not sufficient; thus, grafts will be taken from the ear or ribs, especially in revision cases. (4)
- Tip refinement – Nasal tip will be refined using cartilage grafts and tip sutures
- Alar base reduction – (if indicated) alar base will be narrowed to improve the size and symmetry of the nostrils

Preparation
Once a surgical plan is finalized, our practice will provide clear, personalized instructions to help you prepare. This preparation phase may include pausing certain supplements or medications that can thin the blood, abstaining from alcohol or nicotine, filling postoperative prescriptions in advance, and arranging reliable transportation home from your procedure.
In some cases, lab work or pre-surgical medical clearance may be required, especially for patients with underlying conditions. Procedure
Asian rhinoplasty is performed in a certified surgical setting. Dr. Zhao will determine the appropriate anesthesia based on the extent of the surgical plan. Options include local anesthesia with oral sedation, or intravenous (IV) anesthesia.
During the operation, Dr. Zhao will make incisions either within the nostrils (closed rhinoplasty) or across the columella (open rhinoplasty), depending on the level of access needed. She will then reshape bone and cartilage, refine the nasal tip, or add structural support using cartilage grafts. Autologous cartilage from the septum or ear is often used to build bridge height or improve definition. Each step is carefully performed to ensure the results appear cohesive and refined. Recovery

Week 2
- Swelling and bruising improve
- Most patients can resume light daily activities within one to two weeks. Strenuous exercise, swimming, and contact sports should be avoided for several weeks until Dr. Zhao gives clearance.
Week 3-6
- Swelling improves gradually, with residual swelling often taking up to a full year to completely resolve. You will receive support from our staff throughout this period.
